Healthcare is difficult to navigate. ASTELUME makes the next step clear.
Healthcare is spread across doctors, specialists, pharmacies, insurance companies, equipment suppliers, portals, phone calls, and paperwork. That leaves people trying to connect the pieces while they are already managing their health. ASTELUME brings the entire navigation process into one shared Journey—so you and your Navigator can see what is happening, what needs attention, who is handling it, and what comes next.
What ASTELUME coordinates
Complex care is a chain of connected processes. ASTELUME helps a Navigator organize, coordinate, document, and follow through on each one—while the client can see what is happening and what comes next.
More than a checklist.Every action stays connected to the person's goal, the responsible party, the current barrier, the supporting evidence, and the next accountable movement.
Keep medication lists, changes, refill needs, pharmacy or provider follow-up, and client-reported concerns visible for appropriate escalation.
Identify human-reviewed options using needs such as network, location, availability, language, and accessibility—without implying a clinical ranking.
Coordinate calls, referrals, records, intake requirements, status checks, and follow-up so outreach does not disappear into a voicemail or fax queue.
Support scheduling, confirmation, rescheduling, transportation or accessibility needs, preparation steps, and the questions or documents needed for the visit.
Connect the equipment need to authorization, supplier outreach, delivery, setup, and verification that the item reached the person as expected.
Track requirements, missing information, submitted evidence, payer status, responsible parties, and the next follow-up needed to keep care moving.
Coordinate the practical work around discharge, recovery, receiving providers, equipment, follow-up care, and a safe handoff into the next setting.
Keep referrals, instructions, approvals, records, and verified updates attached to their purpose—not lost in a generic file cabinet.
Make delays and interruptions visible, show who can resolve them, record their impact, and create a clear route back on track.
Keep client updates, internal coordination, decisions, reminders, and next check-ins connected to the Journey so nothing depends on memory alone.
ASTELUME supports care navigation and coordination. It does not prescribe, diagnose, change treatment, or replace a licensed clinician's judgment.
Why ASTELUME
The problem is rarely just finding information. It is keeping dozens of moving parts connected long enough to reach the next meaningful outcome.
Without a navigation home
With ASTELUME
One Journey. Two connected perspectives.
The client and Navigator are looking at the same Journey for different reasons. ASTELUME gives each person the information, actions, and visibility appropriate to their role.
From the client's perspective
ASTELUME gives the client a calm, plain-language view of their care-navigation experience and a direct connection to their assigned Navigator.
From the Navigator's perspective
ASTELUME gives the assigned Navigator an accountable operating workspace for coordinating the non-clinical work between care moments.
One platform. Two service models.
ASTELUME keeps the person's Journey at the center while giving each Navigator the context, boundaries, and next steps needed to move care forward.
For individuals & families
Private-pay clients work directly with a Perscira-hired Private Navigator in a personal, protected ASTELUME Journey.
For organizations
Organizations purchase Perscira for their programs. Their contracted Navigators use ASTELUME to manage assigned Journeys within the organization's program, policies, and service model.
How ASTELUME works
A private-pay client begins with a consultation. A sponsored member begins through their organization's program. The Navigator learns the priorities, pressures, and immediate needs.
Appointments, referrals, authorizations, equipment, barriers, decisions, and supporting evidence are organized around one shared path.
The assigned Navigator coordinates the work, follows up with responsible parties, records what changed, and keeps the person informed.
Everyone with permission can see the current movement, what is waiting, who owns the next step, and how the Journey has changed over time.
ASTELUME does not replace doctors, treatment, or clinical judgment. It gives the navigation work between care moments a clear operating home.

Perscira gives each organization a governed operating environment. Contracted Navigators see the Journeys assigned to them, supervisors see the work they are accountable for, and program leaders gain appropriate operational visibility.
ASTELUME does not turn an organization's workforce into Perscira employees—and it does not give Navigators broad access across organizations.
Organization demos are guided and separate. Perscira will email or send an organization-specific demo link so decision-makers can review the contracted Navigator workflow, supervision, program visibility, and access model.
Trust by design
Every experience reflects who the person is and what they are authorized to do.
Navigators work from assigned Journeys—not an open directory of people.
Clients, payers, supporters, and workforce members receive distinct permissions.
The experience is designed for clarity across devices, abilities, and moments of stress.
Access is part of coordination
ASTELUME is being designed to recognize language, accessibility, communication, and support preferences as part of the Journey—not as an afterthought.
Clients can record language preferences and request interpreter or translated material support for coordinated care activities.
Mobility, hearing, vision, communication, transportation, and other access needs can be connected to the people coordinating the Journey.
Status, ownership, waiting, and next actions are explained directly so clients do not have to decode internal healthcare language.
Find your way in
Private-pay clients, sponsored members, organization teams, and Perscira Private Navigators will each enter through secure, identity-aware access.
